SOCIAL SERVICES
Our social workers help find solutions for older adults and families to address the personal, social, and environmental challenges that come with aging. Their priority is to maintain and enhance the quality of life of our clients.
They can help clients and families identify and implement support services in the community, guide them through the transition from home to assisted living or nursing home, and provide education and support to patients and families when an indvidual is experiencing cognitive decline. They also can assist in connecting individuals with entitlements and financial assistance, as well as help complete advanced directives.
Our social workers additionally provide individual psychotherapy services to patients living at home and in nursing homes.
